Collecting all 21 gems gets you an extra scene after the true ending, after which the If story and Gallery are unlocked. If you have, you have the option to get that or the true ending. If you havent fulfilled a certain requirement, youre forced onto another bad ending. Choosing to 'Save father' leads to a potentially better ending.
